If you are scheduling your Pre-Operative clearance for surgery please click on the button below.

• Select which location you will be seen.

• Under "choose a class" select pre-op (which is the pre-op tab with a stick figure and chalkboard). 

• Then select location again.

• Then click on the date and time that works best for you and fill out the requested information.

• Once completed click on the print forms         then at the bottom of that page you can select and print the new patient forms pdf. 

• If you are unable to print out the forms we ask you check in 15 minutes early so you can fill out the new patient forms.
